I have been battling a similar situation with AT&T lately.  They have 
begun analyzing e-mail traffic to their customers and calculating a 
ratio of spam (or what they define as spam) to good mail.  If your 
server goes over some arbitrary percentage of spam vs. good mail they 
block your IP entirely.  So, now you get whole companies suddenly 
losing their e-mail stream because some wise-ass at AT&T believes 
they are "helping" their customers... <grumble>.
