Actually, there are varied ways to run the outhaul line.  The third pic shows the outhaul line coming back to the cleat.  The second photo shows the outhaul going "down through the second hole" and being held by the cam cleat.  This is probably the preferred method and is how mine is run.  The car is allowed to move, but mine tends to stay in place unless I manually move it.  And yes, the topping lift is generally kept slack when sailing, but some people don't worry about it too much.  As long as when you tighten down on the boom that you're not loading the topping lift.  Getting a smooth rollout of the main depends a lot on boom angle.  A slight downward angle works best on my boat. - rob

Hey Rob...the third pic with the Doyle sail is the setup I have...always wondered what that second hole at the end of the boom was for. Think this will help a lot. Notice the topping lift is taut...don't you slacken this?  Also, when unfurling the main with this setup..what works to give a smooth rollout? Also, again, does the car stay where you place it?
